You will need to complete or submit the following as part of your application. Furthermore, you will be required to have all these items saved as one (1) pdf file and to upload the file in order to complete and send/transmit the Application Form:

- Copy of your High School Certificate/Diploma or College Degree
- Copy of your current Driving Record from MVA
- Copy of your Social Security Card
- Your PPD/negative Chest Xray Result
- Your CMT Certificate
- Your Blood Borne Pathogen Certificate
- Your current MANDT Training Certificate
- Abuse & Neglect Training Certificate
- CPR & First Aid Training Certificate/Card
- Copies of Certificates for the DDA Mandatory Core Courses & Trainings
